Open an Account
To open a Crestpoint account, call 314 849 7773. Once your account is
established/credit approved and your purchase order is placed, we will
confirm your order ship date. We ship, your specified carrier, DHL,
UPS, Federal Express and US Mail. If not completely satisfied with
your purchase, contact us for a Return Goods Authorization Number (RGA)
to return unused merchandise within 30 days. Handle product with care,
a refurbishment/restock charge may apply. The following items will not
be accepted for credit: custom instruments, discontinued items,
sterile/disposable products, specialty items, cannulas and sale items.

schedule. Contact us for pricing, prices are subject to change without

Payment Terms
Invoices are Net 30 days. We accept VISA, MasterCard, American
Express, check and wire transfer.
Shipping and
Handling
Shipments are by your courier of choice; DHL, Federal Express, UPS and
US Mail. Shipping and handling charges will be added to your invoice.

for life against defects in materials and workmanship.
Repairs
Contact us for a Repair Goods Authorization Number (RGA). Pack and
protect the device in proper packaging. Ship device, prepaid shipping,
via a traceable, insurable method to Crestpoint Management Ltd., 9100
Watson Road, Suite 100, St. Louis, MO 63126. Upon receipt of your
device, we will advise you with price for repair and estimated return
ship date.
Returns
Contact us for a Return Goods Authorization Number (RGA). Crestpoint
accepts unused merchandise for credit and in the original package.
Credit issued for all items returned within 20 days after invoice
date, excluding any expedited freight charges. Returns received after
20 days are subject to a 10% restocking fee, plus any requested
special expedited freight charges. When returning items for credit to
your account, ensure to pack and protect the device in proper
packaging. Ship device, prepaid shipping, via a traceable, insurable
method to Crestpoint Management Ltd., 9100 Watson Road, Suite 100, St.
Louis, MO 63126. The following items will not be accepted for credit:
Custom instruments, discontinued items, sterile/disposable products,
specialty items, cannulas and sale items.
Investigational Devices
Certain products featured in the various websites may not be available
if the product has not received marketing/clinical approval. For
